In case of prisoners transferred to another place of internment, the form will, when filled up, be transmitted with the prisoners to the Commandant at the prisoner's destination, and by him to Prisoners of War Information Bureau, 49 Wellington Street, W. C.
In case of prisoners released or deceased, one copy only of this form should be prepared and transmitted at once to the Prison ers of War Information Bureau.
